Since 1997, Kazakhstan has actively started in health care reform for the implementation of family medicine as a more economical and effective method of primary health care. Reform was intended to reorganize the polyclinic system and creation of family health clinics (FHC). Instead of address in various places to seek health care adults and children will be able to get all the primary health care from one health care provider.
From September 1998 there was an order number 484 Health Committee "On the organization of teaching and clinical, outpatient clinics" in connection with which the Department of polyclinic therapy of SSMA was reorganized into the Department of Family Medicine and work has begun to build educational and clinical family medical clinic.
Decree of the President of the Republic of Kazakhstan – State program "Health of the Nation" on November 16, 1998 legislated opening clinics at medical academies, and was provided for equipping them with modern equipment and qualified personnel for the implementation of primary health care. Order number 500 of September 23, 1998 based on international experience, provided the model provisions on training and clinical, family medical clinics and served as the basis for the organization of Training Institutions family outpatient clinic number 12 "Medical Academy".
At Semipalatinsk State Medical Academy first in Kazakhstan established its own medical clinic – CEC EKR CRP, which included a December 5, 1998 as an independent legal entity established Training family outpatient clinic number 12 "Medical Academy". In the creation of a medical clinic SSMA had tremendous help Akim of East Kazakhstan region, and in Semipalatinsk – Akim of city. With constant attention and support of the East Kazakhstan Health Management and Urban Management Semipalatinsk created Clinical Training Center, which is proud of any of the medical schools of the Republic of Kazakhstan.
This Model Training Center general practitioners, is a new step for improving the quality of undergraduate and postgraduate training of doctors. With modern building types and good equipment, the faculty of the department internship GP and staff TFHC–12 "Medical Academy" can and must make a contribution to improving the quality of health care provision. Team teaching and clinical FHC was represented by a group of doctors – assistants who had previously worked on of polyclinic therapy of, pediatrics, obstetrics and surgery. Organization of the department of family medicine has been associated with serious critical period in health care reform in our country. Currently it has a rich and interesting history. In the following years many events in the field of health were directly related to the department, and these changes leave their mark on its status and work.
Department of Family Medicine at the Semipalatinsk Medical Institute organized on the basis of internal medicine department number 3 (in 1987 renamed the Department of polyclinic therapy ) City Central Hospital.
The first head of department became in November 1985 c.m.s. Adilgozhin Social Abdrakhmanovich who was born in 1939. He began his career with the September 1964 as tuberculotherapist in the Aksuat district hospital. In 1965 he became chief physician of regional tuberculosis dispensary. In 1981 successfully defended his thesis on "Methods and effectiveness of TB control in intensive livestock remote area of the Kazakh SSR". Author of 80 scientific articles in medical journals. Since 1988 worked as the chief physician of the regional TB dispensary number 2.
Later in 1988 the head of the department was MD, professor Yuriy Hermanovitch Gajewski, who is currently working at the Department of Internal Medicine Medical Faculty Novgorod city (Russia). Y.Gajewski was born in November 25, 1938, in Baku. Graduated from the Chelyabinsk Medical Institute in 1962. As a doctor of the district hospital he defended his thesis in 1968. In Semipalatinsk Medical Institute has worked since 1970. Awarded the rank of associate professor in 1981, the title of professor in 1990. Professor Y.Gajewski has known in Kazakhstan and beyond as great cardiologist, the author over 200 scientific articles and monographs. He is one of the first authors algorithmic principle diagnosis of cardiovascular system diseases. Author of the famous in CIS monograph on algorithmic differential diagnosis of heart murmurs. Y.Gajewski were first initiated studies of autonomic regulation of the cardiovascular system in health and in various diseases, especially coronary heart disease and hypertension. Also, he was the first in Kazakhstan technique has been introduced 24 – hour Holter ECG daily monitoring. He is the supervisor of doctoral and master's theses. The department under his leadership were improved methods of diagnosis and treatment of medical disorders in an outpatient setting. Introduced new scheme of patients examination, developed a system of continuous outpatient counseling. His disciples are Professor Victor R. Weber (currently Rector Novgorod University), Professor Rubanova Marina Pavlovna (the chief physician of the Novgorod region ), associate professor Shelekhova Lyudmila Innokentevna (works at the Institute of Medical Education Novgoroda city), Associate Professor Karibaev Kairat Rakhmanievitch (the Head of the Department of Therapy our university) and others.
Since the founding of the Department of polyclinic therapy by experienced teachers with extensive teaching stage, doctors of high category Abilmazhinova Bayan Abylkhairovna and c.m.s. Zhukova Tamara Georgievna, Demidova Valentina Ivanovna, Faerman Yakov Ilyitch.
In 1998, at the Department of polyclinic therapy was formed in to Department of Family Medicine, where currently headed by Professor Maira Zhaksimanovna Espenbetova.

The first group of the department of family medicine and endocrinology at teaching clinical FHC was presented by the Head of the Department, Professor Espenbetova M.Zh., lecturers: Kurumbayev R.R. and Kerimkulova A.S., assistants: c.m.s. Zhukova T.G., Abilmazhinova B.A., Khaibullina A.I., Zgotova N.S., Drozhzhinova L.S., Terekhova T.I., Turdunova G.S., Nurpeisova R.G.
Throughout the years, at the Department worked specialists who were not indifferent to the fate of their students and patients.
